Frequently Asked Questions

Answers to the questions we are asked most often about booking a vaccination appointment.

Yes. Search for a clinic, choose an available date and time, then complete the consent form online before you arrive.

No. Booking is open to everyone — no login or account is required. You only need to complete the consent form.

Please bring a photo ID and your insurance card if you have one. You do not need to print your consent form; we already have it.

No. Insurance is not required. If you do have insurance, adding your details on the consent form helps us process your visit faster.

A parent or legal guardian must complete and sign the consent form for anyone under 18, and should accompany them to the appointment.

The vaccination itself takes only a few minutes. You will then be asked to wait 15–30 minutes for observation before leaving.

The consent form includes screening questions covering these. Answering “yes” does not prevent you from being vaccinated — it simply means a healthcare provider will ask a few follow-up questions.

Please contact the clinic directly using the phone number shown on its listing, and they will help you reschedule or cancel.

Your details are used only for your vaccination record and are shared with the MIIS vaccine registry as required. They are never sold or used for marketing.
